Fine lines may be the first age-related change you notice in the mirror, but a softening jawline and a loss of lift in your cheeks point to something happening deeper: a decline in the collagen that keeps your skin firm.
Fotona TimeWalker offers a compelling solution. The platform pairs two laser wavelengths, Er:YAG and Nd:YAG, and uses four treatment modes to tighten and rejuvenate skin nonsurgically. A single session addresses several layers of the face at once, from the deep support structures to the skin’s outer surface, earning TimeWalker its reputation as a “nonsurgical facelift.”
What Is Fotona TimeWalker?
Fotona TimeWalker is a laser platform made by Fotona. It combines two lasers in one device. The Er:YAG laser works near the surface of the skin, and the Nd:YAG laser reaches deeper tissue.
Fotona4D is a treatment protocol performed with the TimeWalker device. It is the name you will often see while researching nonsurgical facelift procedures.
What Separates a Real Nonsurgical Facelift From a Marketing Term?
A treatment often earns the name “facelift,” surgical or not, when it helps to smooth and firm the skin. Although a surgical facelift can produce the most dramatic changes, there is an increasing number of nonsurgical devices that can improve mild to moderate skin laxity. Some are more effective than others.
The American Society of Plastic Surgeons identified laser skin resurfacing as its fastest-growing minimally invasive category in 2024, with procedures up 6 percent to more than 3.7 million nationwide. That growth is not limited to the United States either: nonsurgical procedures overall reached 28.2 million nationwide in 2024, and nonsurgical skin tightening ranked among the five most-performed procedures worldwide last year, according to ISAPS’s Global Survey.
That growth explains why the word “facelift” gets misused so often. Once a category starts trending, marketing tends to borrow its vocabulary faster than the underlying science can back it up, and a radiofrequency wand or an at-home LED mask might call itself a “laser facelift” without reaching a depth sufficient to stimulate new collagen. It’s critical to separate what is actually effective from what is marketing hype.
TimeWalker‘s dual-wavelength design allows it to make the “nonsurgical facelift” claim more honestly than competing technologies. It targets different tissue depths and performs two distinct jobs simultaneously.
How Does the TimeWalker Laser Work?
TimeWalker pairs an Er:YAG laser and an Nd:YAG laser, each calibrated to different depths, speeds and temperatures. Over one session, the laser is used to:
- Warm connective tissue from inside the mouth through an intraoral applicator, lifting the mid-face from within
- Target the dermis to stimulate collagen production
- Resurface the outer skin layer to refine texture and tone
What Are the Four Treatment Modes of Fotona4D?
Each “D” in Fotona4D stands for a separate laser mode. Your provider uses all four in one appointment, adjusting the settings for your skin as the session goes on.
SmoothLiftin™
The treatment begins inside your mouth. The Er:YAG laser gently heats the tissue lining your cheeks and lips, with the aim of tightening existing collagen fibers. The result is an overall improvement of elasticity in the treated tissue, as well as a plumping effect in the nasolabial folds, much like a filler.
FRAC3®
Next, the Nd:YAG laser delivers very short pulses of energy across your face. This nonablative modality improves deeper imperfections to complement the effects of the SmoothLiftin™ mode, as well as restore youthful texture and clarity.
PIANO®
PIANO uses much longer Nd:YAG pulses to warm the deeper layers of your skin. This deep heat stimulates collagen production, which improves firmness and elasticity. Much of the tightening effect comes from this step.
SupErficial™
The session ends with a very light Er:YAG peel that smooths the outermost layer of skin and leaves your complexion looking brighter.
Who Is TimeWalker For?
TimeWalker is a good fit for patients dealing with early structural change rather than significant sagging. You may be a candidate if you have:
- Softening along your jawline
- Deepening of your nasolabial folds
- Skin that’s lost some bounce but not real volume or support
- A preference for gradual, cumulative results over a single dramatic change
TimeWalker is not a substitute for surgery once skin has stretched well past its natural elasticity. At Ambiance MD, your candidacy is determined by Dr. Lisa Allen, who assesses your skin thickness, elasticity, bone structure and fat distribution before recommending a treatment plan.
TimeWalker vs. a Surgical Facelift: Different Tools for Different Patients
A surgical facelift and TimeWalker address similar problems in different ways. The right treatment approach for you depends on various factors:
- Your tolerance for downtime: mild redness that fades within a few hours, compared to one to two weeks of downtime
- Your desired results: gradual, cumulative tightening as new collagen builds, compared to an immediate, dramatic change
- Desired results longevity: maintained through periodic follow-up sessions, compared to results that can last a decade or more
- Cost preferences: a lower per-session cost spread across a series, compared to one larger upfront investment
What Happens At a TimeWalker Consultation?
Dr. Allen examines the areas you are concerned about. Beyond the physical exam, she will also ask about:
- Your current skincare routine
- Sun exposure history
- Any previous treatments, including chemical peels, microneedling or injectables, since these can all affect how your skin responds to laser energy
From there, she maps out which of the four TimeWalker treatment modes your skin actually needs. Some patients benefit most from the full Fotona4D protocol, while others get better results from a partial treatment focused on one or two layers, paired with something like collagen-focused skincare or a separate treatment for volume loss that TimeWalker is not designed to address.
How Should I Prepare for My TimeWalker Treatment?
Avoid sun exposure, tanning beds and self-tanners for several weeks before your appointment. Stop using retinoids, exfoliating acids and other potentially irritating products a few days beforehand unless you are told otherwise.
If you get cold sores, tell us before your visit. You may be given an antiviral medication as a precaution. On the day of treatment, arrive with clean skin and no makeup.
What Does a TimeWalker Session Feel Like?
There is no anesthesia, incision, or sedation involved in a TimeWalker treatment. You will be reclined in a treatment room, wearing protective eyewear, while a provider passes the laser over the treatment area. Most patients describe the sensation as a building warmth rather than pain. A session runs anywhere from 30 minutes to an hour.
Following the treatment, there is little in the way of aftercare or restrictions to abide by. Apply sunscreen and a gentle moisturizer. You can resume normal activities immediately following your appointment. Some patients notice mild warmth or light redness in the treated area for a few hours afterward, which is the extent of most people’s downtime.
Is TimeWalker Safe?
Fotona4D is considered a low-risk treatment when a trained provider performs it. Redness, warmth and mild swelling are common for a few hours to a day afterward. Because SupErficial is a light peel, you may also notice faint flaking for a few days.
The protocol is considered appropriate for all skin types, but Dr. Allen will still review your skin before treatment to ensure it’s a safe choice for you. TimeWalker is not right for everyone. Dr. Allen may recommend waiting if you have an active skin infection or cold sore, have recently taken isotretinoin, use medications that increase sun sensitivity or have a recent tan.
How Does TimeWalker Compare to Other Nonsurgical Skin Tightening Treatments?
There are now many devices that promise to tighten skin without surgery. Each uses a different form of energy, with differing degrees of efficacy and discomfort.
Ultherapy
Ultherapy uses micro-focused ultrasound to heat the deeper layers of the skin, which promotes collagen growth and lifts the underlying tissue. Discomfort has historically been one of its main drawbacks. Some patients ask for a local anesthetic beforehand. Some patients have mild redness for an hour or two afterward, but overall recovery is quick, and many patients are satisfied with their results after a single session. Annual maintenance treatments are often recommended.
Morpheus8
Morpheus8 combines microneedling with radiofrequency energy. Fine needles send the energy into the skin, reaching up to 4mm deep on the face. Topical numbing is applied for about an hour beforehand, and some practices also use local anesthetic injections. Morpheus8 involves more visible recovery than the other options, typically two to five days of redness, mild swelling and scabbing. Most patients need a series of three sessions spaced four to six weeks apart.
Thermage FLX
Thermage uses monopolar radiofrequency energy to heat the deeper layers of the skin while a cooling mechanism protects the surface. There is a brief burst of heat each time the treatment tip touches the skin, followed by cooling. The device also uses vibration to improve comfort. Most patients have little or no downtime beyond temporary redness. Thermage delivers results a single treatment, and many patients repeat it once a year to maintain their results.
How Long Before You See Results?
Collagen doesn’t rebuild overnight. Here is the general timeline most patients can expect from TimeWalker:
- 1 to 2 weeks: improved tone and texture become noticeable
- Ongoing months: tightening continues as new collagen forms
- 3 to 5 sessions: the typical full treatment plan, spaced weeks apart
- Every few months: a single follow-up session to maintain results once the initial series is complete
