Med Spa vs. Dermatologist: When to See Each

Dermatologist or aesthetic provider? At some point, you’ve probably wondered which one you should see for your concerns. Perhaps you have a mole that looks different than it used to. Or a wrinkle won’t budge no matter what cream you try. Or stubborn fat that sticks around even though you’re doing everything right. Picking the wrong provider can waste your time, your money or, in some cases, delay the care you need.

Here’s a simple rule of thumb: see a dermatologist when you need a medical diagnosis, and see an aesthetic specialist when you have a cosmetic goal. Most concerns fall clearly into one category or the other. For those that don’t, an experienced doctor can point you in the right direction.

At Ambiance MD Medical Spa in Green Bay, WI, Dr. Lisa Allen oversees every treatment plan and personally performs many procedures. Patients often come to us with this question, so we’ve laid out how the two specialties differ, why the training of your med spa providers is so important and where to go for the concerns we hear about most.

Dermatology and Aesthetic Medicine Focus on Different Aspects of Skin Care

Dermatologists are medical doctors who complete a residency dedicated specifically to the skin, hair and nails. Their core work is diagnostic and medical, such as biopsying a suspicious mole, managing eczema or psoriasis flares, treating skin infections and screening for skin cancer.

Many dermatologists also treat cosmetic concerns. However, in a practice built around medical diagnosis and treatment, a frown line or dark spot will understandably take a back seat to a suspicious spot.

Aesthetic medicine primarily addresses cosmetic concerns. Instead of assessing what’s wrong with the health of your skin, it asks what you’d like to see in the mirror and creates a plan to get you there safely. This takes extensive knowledge of facial anatomy, skin characteristics, proportion and balance, and how skin, fat, muscle and bone change with age.

Aesthetic providers can improve conditions like acne and rosacea, but what sets them apart from dermatologists is the artistic sensibility they bring to their treatments. Many also incorporate body-focused or wellness services into their practice, which would be out of place at a dermatologist’s office.

What “Med Spa” Means, and Why It’s Important to Choose Wisely

The term “med spa” covers a surprisingly wide range of businesses. Med spas in the U.S. grew nearly sixfold between 2010 and 2023, from about 1,600 locations to more than 10,000, and 70% have no affiliation with a physician practice, according to a 2026 report from the American Medical Association. The same AMA report found that 36 states have no regulations specific to med spas at all.

That is exactly why your choice of med spa carries so much weight. Ambiance MD is led by Dr. Lisa Allen, a board-certified family medicine physician. She completed her residency at Texas A&M Health Science Center, where she was named Outstanding Third Year Resident and received the Arthur P. Gold Foundation’s Gold Humanism Award in 2010. She then spent seven years practicing family medicine in hospitals and clinics across Texas and Wisconsin before opening Ambiance MD in Green Bay in 2017. Today she’s a member of the International Association for Physicians in Aesthetic Medicine and the American Med Spa Association, along with the American Headache Society, the International Headache Society and the American Migraine Foundation.

Listening is the heart of Dr. Allen’s approach. She started her own independent practice so she could take time with each patient, build trust and work toward results as a team. There is no substitute for a physician-led medical spa with a lead provider who actually works in the business every day.

Treatments at Ambiance MD You Won’t Typically Find at a Dermatologist’s Office

Dr. Allen practices general medicine in addition to aesthetics, so Ambiance MD offers care that falls outside most dermatology and cosmetic practices.

Medical Weight Loss

Weight management is outside the scope of dermatology. At Ambiance MD, Dr. Allen offers medical weight optimization that can include GLP-1 medications. Before designing your treatment plan, she reviews your medical history, current medications, thyroid function, metabolic health and cardiovascular risk factors. Some patients lose weight and still have pockets of fat that won’t respond to diet or medication. For those areas, body contouring offers targeted fat reduction.

Awake Liposuction

Awake liposuction removes fat through small incisions using local anesthesia instead of general anesthesia. The treatment area is numbed with tumescent fluid, a solution of saline, lidocaine and epinephrine. You stay awake throughout the procedure, and the fluid also helps reduce bleeding and bruising. Because there’s no general anesthesia, you avoid the grogginess and nausea that follows. Most patients return to desk work within a few days.

CoolSculpting® Elite

CoolSculpting Elite is a nonsurgical fat reduction treatment that uses controlled cooling, called cryolipolysis, to destroy fat cells. Over the following weeks, your body naturally clears away the treated fat cells. There are no incisions, needles or anesthesia, and you can return to normal activities right after your session. Treatable areas include under the chin, the jawline, the abdomen, the flanks, the thighs, bra fat, back fat, the area beneath the buttocks and the upper arms.

Botox® for Migraines and Jaw Clenching

Botox has been FDA-approved since 2010 to prevent chronic migraine in adults who have headaches on 15 or more days a month. Dr. Allen is a member of the American Headache Society, the International Headache Society and the American Migraine Foundation. That training helps her recognize when frequent headaches, jaw clenching or neck tension may respond to neuromodulator treatment.

O-Shot® and P-Shot®

The O-Shot and P-Shot use platelet-rich plasma (PRP) derived from your own blood to address sexual health concerns. A small amount of blood is drawn and spun in a centrifuge to concentrate the platelets. The PRP is then injected into intimate tissues after a numbing cream is applied. Platelets contain growth factors that are thought to support tissue repair and blood flow.

The O-Shot is used for women with concerns such as decreased sensitivity, difficulty with arousal, vaginal dryness or mild stress urinary incontinence. The P-Shot is used for men with concerns about erectile function or sensitivity.

Questions to Ask Before You Book Anywhere

Whichever door you walk through, it’s important to get informed before you sit down in a treatment chair. Although it’s exciting to plan your treatment, it’s never a good idea to skip vetting the provider and the practice.

Ask these before you commit to any dermatologist, med spa or injector:

  • Who will perform my treatment? Get a name and credentials that you can double check.
  • Is that person board certified, and in what specialty? If you are seeing a physician, verify their credentials through the American Board of Medical Specialties.
  • What happens if I have a reaction or complication? Ask who is on site and how quickly you can reach them.
  • How long is my consultation, and what does it cover? A rushed five-minute visit before your treatment is a red flag, not a convenience.
  • Will my medical history factor into my treatment plan? Any ethical provider should consider your medications and health history before recommending a plan.
  • Can I see verifiable before-and-after results from this specific provider? Ask for examples specific to that provider.

Using a Dermatologist and a Med Spa Together

Many patients get the best results by maintaining a relationship with each provider. A common arrangement is an annual skin exam with a dermatologist, with aesthetic treatments at Ambiance MD throughout the year.

If you see a dermatologist and a med spa, keep each one informed about your treatments with the other. Tell your med spa provider about any prescriptions from your dermatologist, especially these:

  • Topical retinoids such as tretinoin, adapalene and tazarotene: These make skin more sensitive. You’ll usually need to stop using them for several days before a peel, microneedling or laser treatment.
  • Isotretinoin (once sold as Accutane): This affects how skin heals. Your provider may recommend waiting after you finish a course or adjusting the type of treatment.
  • Oral antibiotics: Doxycycline and other oral antibiotics increase sensitivity to light, which is a concern for IPL and laser treatments.

Dr. Allen will also take your dermatology visits into account. If she notices a mole or spot that looks suspicious during your visit, she will suggest seeing your dermatologist for evaluation.
